PSR - Registration Form

PSR Registration (Grades PreK-8)

Classes are offered Sundays from 9:00 a.m.-10:15 a.m.
Students attend for 1 session, 1 day a week.

Tuition:
$20.00 per child for Stories & Scriptures
$40.00 each for the first and second K-8 student in a family
$80.00 maximum for families with 3 or more K-8 students
$50.00 additional for each student in a sacramental year. 
(A sacramental year is one in which a student will be learning about
and preparing to receive First Reconciliation, First Eucharist or Confirmation.)

We will be taking photographs and/or digital images of children randomly throughout the school year for publication on our website or other media purposes (i.e.- newsletter, bulletin, etc.)  Your permission is needed in case your child's photograph is used.

I hereby give my consent to St. WIlliam Parish for photographs of my child to be used for website or media purposes.  I further state that I have the right to grant this permission, as I am the child's parent or legal guardian.
