I hereby grant permission for the following named child to speak with an adult member of the St. William Parish during the  2025-2026 Catechetical Year under the guidance of his/her parents, the PSR Catechist, Ms. Lloyd, and/or Mrs. McCue.

In consideration of the child being allowed to participate in the St. William Prayer Partner Program, on behalf of the child, my spouse and myself, I hereby assume all risks in connection with the Program and I further release the Bishop of Youngstown, the Roman Catholic Diocese of Youngstown, St. William Parish, and the pastoral staff, employees, and volunteers thereof from all claims, judgement, liabilit for injury or damage that the child or his/her estate, myself or my spouse ever had, now as, or may have due tot he child’s participation in the St. WIlliam Prayer Partner Program, including all risks connected therewith, whether foreseen or unforeseen.

I fully understand what the Program involves, and I understand that I should call the catechetical administrator with questions about the Program.
